All deposits paid ($300) are NON-REFUNDABLE!

A deposit can be transferred one time for free to a different date. The second transfer will incur a $75 transfer fee.

If your package is paid in full and you cancel your package more than 7 days before your service date, you will be refunded your FULL balance minus the non-refundable deposit.

If your package is paid in full and your procedure is canceled by the surgeon/physician less than 7 days before your service date, you will be refunded your FULL balance minus the non-refundable deposit (a letter from the surgeon is required).

If your package is paid in full and you cancel your package less than 7 days before your service date, you will receive a refund of the balance minus the non-refundable deposit and a 25% surcharge.
